1. Pay $129 then Book New Patient
– After payment $129 register in Health Engine
– New patient, Agree, New phone
– choose any Dr, Guest, your Name, mobile
– email, address, dob, Medicare
2. Health Engine email confirms Phone appointment
– click New Patient Form add extra medical info for consult
– medical condition, medicine tried, risks to consent
– not bipolar, schizophrenia
– part of opiate treatment or rehab
– not pregnant, planning pregnancy or breast feeding
3. All patients apply for Myhealth Record + allow access
– or provide GP letter with medical history, medicines
– Drs + NPs review conditions + medicines for risks

Internationals need Individual Health Identifier number
– add IHI to form or email IHI name, number + photo id [email protected]

You need your passport with a valid Australian visa or Australian driver’s licence
- Sign in to myGov. if you don’t have myGov account
- Select ‘Linked services’ in the menu
- Select ‘Individual Healthcare Identifiers service’ from list
- Follow the steps to apply
4. Pay then book 1st Month Review
3-7 days in advance as Acacia clinic is busy
– Next Review 2-4 months when scripts finished
– all expire 6 months
